Permanent Full Time Position
- Conduct client onboarding assessments & reviews to ensure services and care is planned, aligned and delivered with consideration to a clients competency, capability and dignity of risk.
- Developing budgets for clients with a health promotion focus.
- Deliver nursing services to the client in their home, via telehealth or within the Dementia Respite Cottage
- Ensure effective management of clinical matters (including primary responsibility for wound care, continence care, nutrition/weight management, medication chart monitoring, high risk medication administration, behaviour management, and nursing assessments/consultations).
- Develop behaviour support plans for clients living with dementia and complex care needs.
- Provide leadership, direction, support, and mentoring to Home Care direct care staff including Enrolled Nurses.
- Act as a point of escalation for clinical queries from the Home Care Coordinators working in Home Care and Site Leads-Enrolled nurses working in the Dementia Respite cottages.

About you
We are looking for people who will be the difference in the lives of our residents and clients.
The successful candidate will have the following skills and experience;
- Holds a Bachelor of Nursing, or recognised equivalent
- Current registration and practicing certificate with the Australian Health Practitioners Regulation Agency as a Registered Nurse
- Current Driver’s Licence (Class Car) and Own registered and insured car
- Current First Aid & Cardiopulmonary Resuscitation (CPR) Certificate
- Proven experience in clinical nursing, delivering high-quality care to older adults with cognitive impairment and complex needs.
- Strong leadership experience in supervising and mentoring direct care staff.
- Skilled in working with clients from culturally and linguistically diverse backgrounds, including Aboriginal and Torres Strait Islander communities.
- Excellent communication and interpersonal skills, with the ability to build rapport and support clients with complex needs.
- Competence in auditing, quality assurance, and compliance with regulatory standards.
- Strong organisational and time management skills, able to work autonomously and coordinate others to meet deadlines.
