Permanent Full Time Position - Mondays to Fridays (8h00 to 16h06)
About the role
The primary function of this role is to manage resident admissions to ensure a smooth transition into aged care. Additionally, the nurse supports the Care Manager, Admissions and Clinical Lead, and clinical staff in managing resident care, monitoring clinical issues, and providing guidance to ensure safe, inclusive, and person-centered practice.
Admissions & Assessment: Facilitate the admission process by collaborating with residents and families to create effective, person-centered care plans. This includes completing necessary assessments for accurate AN-ACC classification and monitoring for reclassification needs.
Clinical Support & Leadership: Provide guidance and mentorship to nursing staff and personal carers. Responsibilities include monitoring care plans, pain charts, and weight documentation; assisting with wound management; and supporting the clinical team with deteriorating or high-risk residents.

About you

The successful candidate will have the following;
- Qualifications: Current AHPRA registration as a Registered Nurse with post-graduate experience (at least 3 years), and current First Aid and CPR certificates.
- Skills & Knowledge: Must possess high-level clinical assessment skills, the ability to analyse data to solve clinical issues, and sound knowledge of Aged Care Standards and AN-ACC funding.
