About the role
The Wellness and Lifestyle Coordinator is responsible for delivering services that empower residents to engage in meaningful activities, improve health literacy, and maintain independence. In this dynamic role, you will connect residents with the community, lead staff and volunteers, and implement proactive wellness strategies to support residents' ongoing quality of life. Key Responsibilities;

Health Promotion: Conduct holistic assessments to understand resident goals and preferences, developing personalised wellness pathways for those at risk of decline.

Person-Centered Care: Foster an inclusive environment by encouraging participation, listening to feedback, and supporting residents in making informed life choices.

Governance & Quality: Ensure full compliance with Aged Care Quality Standards, drive continuous improvement initiatives, and mentor staff, students, and volunteers.

About you
We are looking for people who will be the difference in the lives of our residents and clients. To be successful in this role, you will have:
Qualifications & Certifications

Diploma or equivalent in Health Promotion, Social Work, Dementia Care, Health Coaching, or Leisure and Lifestyle.

Current Driver’s License (desirable).

Senior First Aid and CPR certificates (desirable).
Skills & Experience

Proven experience conducting person-centered assessments (e.g., QUALID, WHOQOL-BREF) and evaluating program outcomes.

Strong digital literacy, including proficiency with Google Workspace. Experience with iCare, Person Centred Software (PCS), or similar aged care management software is advantageous.

Sound communication, time management, and practical problem-solving skills, particularly regarding resident risk and independence.
Knowledge

Strong understanding of contemporary best practices in aged care, dementia care requirements, and values-based goal setting.
