Make an Impact Where It Matters Most: In the Community
Are you an experienced Registered Nurse looking for autonomy, purposeful clinical work, and a genuine work-life balance?
At Southern Cross Care, we believe older Australians deserve to live independently, safely, and with dignity in their own homes and communities. In this diverse, multi-program role, you will lead clinical delivery across our Home Care, Dementia Respite Cottages, and Restorative Care (including TCP) programs.
If you thrive on clinical problem-solving, building genuine one-on-one client connections, and guiding direct care teams to deliver best-practice care, this is the role for you.
What We Offer You
-
Maximise Your Earnings: Salary packaging up to $15,900.
-
Bee Well Program: Exclusive corporate discounts on groceries, travel, and private health insurance, plus comprehensive mental health and wellbeing support.
-
KM Reimbursement: Paid travel between community visits.
Key Responsibilities
-
Clinical Assessment & Case Planning: Conduct comprehensive onboarding assessments, restorative care goals, and reviews through a preventive health and wellness lens.
-
Specialised Clinical Delivery: Oversee complex community clinical needs, including advanced wound management, continence care, high-risk medication administration, and nutrition/weight tracking.
-
Dementia & Behaviour Support: Develop, implement, and review evidence-based Behaviour Support Plans (BSPs) to support clients living with dementia in respite and home settings.
-
Team Leadership & Clinical Escalation: Mentor and guide Enrolled Nurses (ENs) and frontline Support Workers; lead clinical orientations, monitor competencies, and serve as the go-to escalation point.
-
Governance & Standards: Ensure documentation and service delivery strictly align with AHPRA standards, the Aged Care Quality Standards, and emerging Support at Home framework requirements.
What You Bring
-
Registration: Current, unrestricted Registered Nurse registration with AHPRA.
-
Qualifications & Certifications: Bachelor of Nursing (or equivalent), current First Aid & CPR certifications.
