About the role
The Care Manager - Home Care is responsible for leading the achievement of high quality clinical and care services of our East Melbourne and Mornington region within the Home Care portfolio, coordinating resources and activities to achieve person centred care for clients/care recipients that is responsive to the clients assessed needs.
You'll work amongst a multi-disciplinary team to provide integrated care that promotes healthy ageing and well-being, social engagement, prevents frailty and avoidable decline and achieves reablement for clients to optimise their ability to live better in their own home.
Key Responsibilities include but are not limited to;
- Ensuring all clients in the region have a current and holistic assessment and a service plan which manages all risks associated with their assessed needs, within their service budget and with a health promoting focus
- Actively engaging with the community within the region, with the aim of promoting Southern Cross Care as a Provider of Choice for Support at Home services to older people
- Acting as an advocate for a client/care recipient to assist with making informed decisions
- Monitoring the quality of comprehensive care planning strategies to ensure the clients quality of life and independence is optimised, while managing risks and duty of care
- Ensuring all assessments and documentation meet legislative requirements

About you
We are looking for people who will be the difference in the lives of our residents and clients.
The successful candidate will have the following skills and experience;
- Bachelor of Nursing, or recognised equivalent and current registration and practising certificate with AHPRA as a Registered Nurse
- Current and unrestricted driver’s licence & own registered and insured car
- Strong organisational and time management skills
- Experience in Clinical Nursing role, including skills and knowledge to be able to provide a standard of excellence in clinical care
- Demonstrated experience in the leadership of direct care staff
- Proficient computer and keyboard skills, including the use of community aged care systems/processes (eg, MyAged Care, ACAT, RAS etc)
- You will be suited to this role if you have a positive mindset and can prioritise well
- Ability to demonstrate integrity, calmness and personal resilience
- Passionate about person-centred care, enabling clients to maintain maximum independence, you will be able to seek creative solutions in your approaches to overcome any barriers
