About the role
To deliver wellness and lifestyle services and activities that empower residents to engage in meaningful activities, be physically active, connect with the community, and participate in wellness programs that align with SCC's "Better for life" model. It also involves contributing to early intervention programs and ensuring resident satisfaction with wellness services.
Key Responsibilities:
- Health Promoting Services: This involves working with staff and practitioners to provide person-centred care, conduct holistic assessments, tailor programs to individual needs, empower residents to set SMART goals, advocate for opportunities, support risk-taking, maintain accurate records, and assist with early intervention programs and the implementation of specific lifestyle programs (SAL and Fringe). It also covers balancing resident independence with potential risks and duty of care, and advising nursing staff of changes in resident behaviour or condition.
- Person Centred People Our Values: This section emphasises treating residents with dignity and respect, listening to their needs, collaborating with health professionals for wellness program design, considering all resident needs (social, cultural, religious, etc.), and empowering residents to make informed decisions for their well-being.
- Aged Care Friendly Environment: Responsibilities include annual program evaluation, reporting risks and incidents (including elder abuse), maintaining resident privacy, and staying updated on best practices for lifestyle activities for older people.
- Governance (Quality Control) & Safe Work Environment (WHS): This covers using continuous improvement systems, monitoring resource use, complying with professional ethics, and working safely (including reporting for duty unimpaired, using WHS equipment, following procedures, and reporting hazards/incidents).

About you

Essential Requirements:
- Qualifications: Certificate IV Leisure and Lifestyle or equivalent.
- Experience: Delivery and implementation of wellness/lifestyle programs for older people, experience in aged care, and completing person-centred assessments.
- Skills: Best practice skills, sound communication (especially with older people), workload management, problem-solving (balancing independence vs. risk/duty of care), physical and emotional ability, manual handling, and proficiency in Microsoft Office and healthcare records software.
- Knowledge: Understanding of supporting residents with dementia, values-based goal setting, appreciative inquiry, motivational techniques, referral pathways, Aged Care Standards (accreditation), AN-ACC application, and contemporary best practices in aged care wellness programs.
- Personal Attributes: Alignment with SCC's culture and values, ability to build positive relationships, calm decision-making in emergencies, collaborative team commitment, integrity, resilience, and commitment to person-centred customer service.
- Desirable Requirements: Experience in developing, implementing, and evaluating wellness and lifestyle programs for older people, and previous experience with iCare, PCS, and Medimap software.
