About the role

Deliver high-quality clinical and care services for clients in home care, focusing on person-centred care and responding to assessed needs.

Key Responsibilities:

  • Manage, Lead & Achieve High Quality Person Centred Care / Health Promoting Services:
    • Conduct client onboardings, reviews, assessments, and budget development to create holistic service plans that manage risks and promote health.
    • Deliver best practice nursing services in the client's home or via telehealth.
    • Monitor and supervise support for daily living, ensuring quality care planning, optimising quality of life and independence, and managing risks.
    • Ensure all documentation meets legislative requirements.
    • Respond to incidents in compliance with policies and legislation, focusing on preventing recurrence.
  • Professional / Clinical Practice & Governance:
    • Effectively manage clinical matters (wound care, continence, nutrition, medication, assessments) and escalate issues.
    • Collaborate with clients, relatives, staff, and other health professionals.
    • Act as a client advocate and support health literacy.
    • Lead and supervise best practice clinical care.
    • Adhere to National Competency Standards for Registered Nurses and the Code of Ethics.
    • Maintain up-to-date professional knowledge and skills and stay informed of healthcare changes.
  • Leadership:
    • Provide effective leadership to direct care staff, including acting as an escalation point.
    • Monitor staff competency and conduct HCA competency assessments.
    • Conduct staff orientation and training under the Care Manager's direction.
  • Safe Work Environment (WHS):
    • Work safely and protect own and others' health and safety.
    • Report for duty unimpaired by alcohol or drugs.
    • Use WHS equipment, obey instructions, and comply with all WHS policies and procedures.
    • Identify and report hazards, near misses, injuries, and maintenance issues.

About you

We are looking for people who will be the difference in the lives of our residents and clients.

The successful candidate will have the following;

Essential Requirements:

  • Qualifications: Bachelor of Nursing (or equivalent), current AHPRA registration, current Class Car Driver's Licence, own registered and insured car, current First Aid & CPR Certificate.
  • Experience: Clinical Nursing role (excellence in clinical care), leadership of direct care staff, working with Culturally and Linguistically Diverse clients/groups (including Aboriginal and Torres Strait Islander Australians).
  • Skills: Sound interpersonal and communication skills (high proficiency in English, effective communication with older people with complex needs), competence in auditing and quality assurance, time management, effective decision-making, ability to manage complex assessments and clinical risk, professional composure under pressure, proficient computer skills (including aged care systems like My Aged Care).
  • Knowledge: Basic knowledge of Aged Care Standards, in-home aged care program guidelines and funding, understanding of clients with cognitive impairments (including challenging behaviours).
  • Personal Attributes: Alignment with SCC's Values & "Better for life" model, ability to build positive relationships, commitment to teamwork, integrity, calmness, personal resilience, commitment to person-centred customer service, commitment to best work, problem-solving approach to risks.
  • Special Conditions: Willingness to use own motor vehicle for travel to clients.
  • Desirable Requirements: Postgraduate qualification in gerontology or a related discipline.
