About the role
The Clinical Coordinator - Registered Nurse plays a key role in supporting the Respite Team to provide a person-centred care within our dementia specific environment, facilitating a holistic approach in supporting increased complexities and behaviour management supports through completion of comprehensive assessment and collaborative care planning.
Key Responsibilities include but are not limited to;
- Lead the SCC Community Respite model of care, delivering high quality, meaningfully engaging care and services to clients.
- Complete comprehensive, person-centred clinical assessments, scheduled and urgent client care plan reviews
- Use evidence based, best practice to work alongside and to support the respite assistants to ensure clients health, behaviour support, medication, continence, exercise and engagement needs requirements are met.
- Respond to client incidents and identify early signs of decline to provide best practice clinical intervention for the client.
- Work as part of a multi-disciplinary team and engage in case conferencing to proactively support clients while including relevant internal or external providers to access appropriate internal and external services that will maximise independence, improve and maintain their health, wellness and quality of life.
- Team engagement to promote service integrity and quality
- Work closely with the Respite Occupational Therapist and Program Manager to promote continuous quality improvement of the service, compliance with aged care standards and support SCC’s organisation goals.

About you
We are looking for people who will be the difference in the lives of our residents and clients.
The successful candidate will have the following;
- Tertiary qualifications in nursing with current registration with AHPRA
- Current Provide First Aid & Provide Cardiopulmonary Resuscitation (CPR) Certificate
- Current drivers license
- 5 or more years of experience working in aged care
- 3 or more years of Dementia care experience
- Clinical expertise in working with community dwelling older adults living with Dementia with challenging behaviours and complex needs
- Experience in the use of community aged care systems and processes such as MyAgedCare, CHSP, ACAT, RAS etc.
- Experience with case management of the holistic needs of clients.
- May require to travel to other SCC cottage locations if required
