About the role
The core function is to complete admissions, ensuring a smooth transition for new residents into aged care, and to support the Care Manager/Admissions and Clinical Lead RN and clinical staff. This includes ensuring accurate AN-ACC classification for new residents and monitoring reclassification needs. The role also supports the Care Manager in daily staff and resource management to meet resident needs.
Key Responsibilities:
- Person-Centred Philosophy and Admissions: Facilitating smooth admissions by listening to residents and families, developing safe and effective care plans, considering diverse needs, supporting informed decision-making, partnering with the Care options team, liaising with health professionals, acting as an advocate, and completing necessary assessments for AN-ACC classification.
- Health Promoting Services: Collaborating with staff to provide proactive, holistic, and person-centred care. This includes providing leadership, guidance, coaching, and mentoring to nurses and personal carers, assisting with high-priority tasks, reviewing progress notes, staying updated through email, monitoring various charts and care contracts, supporting referrals, assisting with clinically deteriorating residents, regularly reviewing and updating care plans (including 6-monthly reviews), monitoring care delivery and evidencing, assisting with wound management, being aware of fall risks, and monitoring risk management strategies.

About you
- Essential Qualifications: Current AHPRA registration as a Registered Nurse with relevant postgraduate experience (3 years guideline), current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009)
- Essential Experience: Previous experience as a Registered Nurse with high-level clinical skills, supervisory, leadership, and educational responsibilities in an aged care environment.
- Essential Skills: Demonstrated best practice clinical management, ability to analyse data and problem-solve, advanced clinical assessment and documentation skills, sound communication skills (including English literacy), ability to communicate with different staff levels, time management, excellent decision-making, negotiation, and problem-solving skills, physical and emotional ability to perform the job, manual handling skills, and computer skills (Google, PCS).
- Essential Knowledge: Sound understanding of health promotion and person-centred care, excellent knowledge of Aged Care Standards, knowledge of AN-ACC funding, and excellent contemporary best practice clinical knowledge in aged care.
- Personal Attributes: Commitment to SCC's Values and "Better for Life" model, ability to build positive relationships.
- Desirable Requirements: Postgraduate qualifications in gerontology, previous auditing and quality assurance experience.
